Appendix 6: Influenza Pandemic Preparedness & Response Plan

Notification of Outside Agencies

As soon as possible after suspicion of an outbreak, the Director of the Infection Control Program at the University of Chicago Medical Center (UCMC) will notify local public health authorities, including CDPH and IDPH. If preliminary investigations support that an incident has occurred, the Director of the Infection Control Program, in conjunction with local public health authorities, will notify federal agencies, including the CDC.

The regulatory agencies will be provided with the following:

  1. the location of the outbreak;
  2. the name and telephone number of contact at the location;
  3. the illness believed to be involved;
  4. the number of people who are sick; and
  5. the steps taken to control the outbreak.

The Director of the Infection Control Program will act as the liaison between the UCMC and CDPH, the IDPH, and/or the CDC. Where applicable, the Executive Director of the University Police Department will act as liaison with law enforcement agencies. The ED will act as liaison with the Chicago Fire Department and Emergency Medical Services.

Requests for prophylaxis and/or medical supplies from the Strategic National Stockpile (SNS) are made through the IDPH Operations Headquarters and Notification Office (IOHNO) as outlined in the State Emergency Medical Response Plan. At the time of State Emergency Medical Response Plan activation, the State Emergency Management Systems (EMS) Coordinator will call the UCMC Emergency Department with information on how to contact IOHNO.
